CAF Database
Having identified additional needs using the Pre-Assessment Checklist, you should register this with the CAF Database, also checking whether a CAF already exists for the child/young person.

If there is no existing CAF, the next step is to complete the CAF and register the outcome with the Database. You keep the CAF (and provide a copy to the parent/carer).
The outcome may be that your organisation is able to meet the needs of the child/young person, or is able to make referrals to other organisations able to meet the needs identified.
If you feel there are additional needs but you are not sure how the needs can be met, you can refer the child/young person to the local Children and Young Peoples Support Panel (CYPSP).
